    If you're really heavy, you definitely can lose fat and gain muscle significantly. Just lift heavy as hell and eat clean.

    mostly BBers get them, because their form on the bench is not very good. PL form is very safe. some PLers rip them when they do STUPID things, like very stupid. otherwise it's fine. joints should be fine unless your form is bad.

    Lifting to failure pretty much makes your form worse and worse => injury. If you bench like 95% of the goons in the gym, don't bench to failure. If you bench PL style, don't bench to failure as PL style gives you maximum weights. So do not flat bench to failure.

    Yes your hips just need to pass your top knee, which is going under parallel. I don't know what link you're referring to but I'm very sure Mark never said to go A2G! Nonetheless if that's what you like to do it's fine too. 400 is a lot! LOTS! A lot of gyms here you can go for a year with 400$!

    Good form = safe! He did say MOSt people, look around in a commercial gym and you'll see his point. Learn form, and no problem!

    Do the startin gstrength workout. Follow it to the letter, don't do funny things to it, you will get strong. And eat :)
