Alot of these are judgement calls, but think outside the box with my suggestions. Honestly, I wouldn't bother unless this was the sexiest girl on the planet, and God himself told me this was my soulmate and I would never get bored with her not even for 5 minutes, but hey, this is your call.
1) No, dont tell her you're schtupping other girls. Subtly make it known that you are wanted and begin using DHV (and negs, if necessary. For definitions, go to the bible link at the bottom of the page and get reading.)
2) No. Unless you want to leave the friendzone and enter Chumpville, where you will become an errand boy and the most you get is a hug for doing stuff for her.
3) Call her when you see fit. Depending on how far in you are, I would start limiting my availability and begin making myself scarce. When you do call, make sure you are in a noisy enough venue so it sounds like you would have to get off at any moment, or if you text, mirror the time it takes for her to send replies to yours. If its hours, dont bother sending it, especially if you know she was just sitting on her @$$.
4) Change the subject. You arent an emotional tampon and you have a cack, not a pus. You dont need to hear it, but dont say those exact words.
Example:
HER: BLAH BLAH, Thomas and I are blah blah blah.
You: I like soup, do you like soup?
Just remember to not lower your value, always raise it and make her see what shes missing.
