I'm Italian and as far as I know, Italian Mafia doesn't even exist anymore outside Sicily... And even there, when it was born and grew, is not as powerful as some decades ago. Now, they limit themselves to extort "il pizzo" and to build houses illegally.
The Mafia is now 3rd in power in Italy and is outrun by Naples' Camorra and Calabria's 'Ndrangheta, which are undoubtedly a lot more powerful. Camorra is nowadays the most powerful since every mafia in the world buys its guns and weapons by it. I read a book about Camorra a couple of months ago and it was impressive.
