I have found that when my skin is moist (like from lotion) it's way more prone to breaking out, so I only use it if my skin gets really dry!

Hey man, this shouldn't be happening if you use non-comedogenic (oil-free) moisturizers. Just make sure it says "Non-comedogenic" on it somewhere.
