OP, do you have any women you like or are attracted to? Do you talk to women often?
One basis point is to get comfortable in talking with women. Make smalltalk and chat with women you see, at work, in the checkout at a store, etc. I often make banter with women at work, or even the security guards in shops. One reason is just to be friendly, the other is that it's practice. It lends to experience in talking with women and people in general.

Serious answer:
1) You keep doing what you were doing that got you the first few dates.
2) You NEVER inquire about the status of your relationship with her. It is whatever it is.
3) When you have sex for the first time with her you do NOT assume it means she is now your girlfriend.
4) She will either inquire about being your girlfriend or her actions will show that she clearly has high interest in you, in which case you do not need to ask any questions about your relationship - just go with the flow.
This stuff is not as easy as it sounds when your goal is for a particular girl to be your girlfriend, which is why people will say spin plates so you don't let your emotions take over.

I don't think directly asking a woman to be a girlfriend is ever acceptable.
it kind of fits into place. once there is mutual attraction and warmth, simply tell her "it seems we're exclusive now, don't you think?" It needs to be subtle, not overt.

Ilikecharlene: NO. You do not do that. She should be the one suggesting that the relationship is exclusive.
