How common are shoe lifts or elevator shoes

The nearest I've ever come to wearing them was with (cowboy) boots with a 2" heel. But I take the attitude that at some point you're going to be naked (or have just come from being naked), and you're going to have to stand toe-to-toe with your woman, so what's the point in deceiving?
