She pretty much wants you to jump to her defense and spruce up her fragile little ego. Witch is bullshyt, men are not attention or validation buttons, don't fall for her petty little games.

I don't know, but agree with her and elaborate that low self-esteem is indeed ugly. Then tell her that the good news is that at least it's cheaper to fix than plastic surgery.
If she starts referring to her looks then say "oh, your mirror must be broken".

Assuming you've already replied to this statement, if she makes the comment again, tell her that you and she have already discussed this and you will appreciate if she doesn't ever mention it again.

When I respond, I usually passively agree and blow it up even more. I'll say "Yep, absolutely hideous." When they give me hell for saying that, I'll usually say "Well, you're the one who said it. If you don't want me to agree, then don't say it." It works quite well.

And she's probably decent looking. As she could have been unlucky and had an accident or born disfigured or disabled!
