What i asked last time i wanted to go exclusive with a girl was:

Me: "Do you want me to be exclusive with you, only dating you?"

Her: "Of course i want, what do you think? Of course i'm not, i'm with you!! Are you dating other girls, or thinking off??"

Naw, you should know if you listen to her. Asking her directly is a really weak move, and in general, if she likes you, the answer is no.
