Just had my Incision and Drainage

Oh God bless that doctor, even though he caused me more pain. So anyways the nurse didnt clean my up so I did it myself with alcohol free baby wipes, they packed my wound and Im pain free now... I have one question, they have to re-pack my guaze and pull the one I have in right now back out, is that process painful?

Andy,

Hey man, I am a medic and have dealt with quite a few abscesses, including some in the anal region.

I'm guessing your's started as "folliculitis" or a clogged and inflamed hair follicle, and eventually developed into a Staph infection?

Generally they respond well to Warm Compresses, and antibiotics.. What are you taking? Cephalexin (Keflex), Augmentin, or Septra/Bactrum? Just curious what they gave you.

What are you taking for pain? Ibuprofen (Motrin) would be preferable, 800mg every 6-8 hours with food/milk. With Motrin you get the analgesic effect (pain relief) along with the Anti-Inflammatory effect after 4-7 days, and finally it will help with the fever you may develop as your immune system kicks into high gear to fight off the Infection. If you only have Acetaminophen (tylenol) aim for 650-1gram every 6 hours. You can max out with 4 grams of tylenol in a day. 3200 for Motrin.

If it doesn't go down with the anti biotics there is a good chance it will need to be I&D'd which is short for Incise and Drain.. You will be numbed with Lidocaine or Marcaine, then it will be lanced and drained. After that it will usually be packed daily, with Iodaform or another type of stringy sterile gauze that allows the wound to heal from the inside out.

DO NOT ATTEMPT TO PLAY WITH IT YOURSELF... LET THE ANTIBIOTICS DO THEIR JOB!!!!

I replied to your other post.. Glad you went ahead and had the I&D done.. I know its a huge relief for the pressure to go away.

As for the repacking, yes it is painful because due to the way the wound heals from inside out, tissue will begin to grow around the gauze and when the gauze is removed it will cause the wound to bleed which in turn makes it continue to heal.

Don't sweat it too hard dude..

Yeah well how many times am I going to have to change to gauze? My mom has been changing the padding around the wound (i know im a big baby) well I cant do it myself im not that coordinated :p anyways, everytime I sit down I get this nasty feeling as if blood is just dripping down from the wound, when im laying down too, then when I change the gauzes around the wound theres alot of blood but nothing actually coming out of the incision, is it normal to bleed like that?

Oh PS: I have to use the bathroom bad! Can I? Will the would be fine and what if I bleed?

Yes its normal for it to bleed. Considering it is on your bottom it will drain since generally we do not spend the majority of our time on our stomachs.

You will be fine man, just use the bathroom, trust me, now that they performed I&D you are on the path to healing.. Just keep it clean and don't "play" with or touch it. And if anyone is touching or changing the packing guaze they NEED to be wearing sterile gloves..

okay im taking care of it pretty well, the doc said my abscess was HUGE and i might have to have an excision? can you please explain that to me im not sure what it is

Hey dude, basically if the wound does not heal properly after the I&D, then the next step is a Excision which would basically be them cutting down into the infected area and removing all the affected surrounding area. It's not as gruesom as it sounds, especially on the butt which is primarily Adipose (fat) and muscle. I highly doubt the infection has spread all the way down through the sub cutaneous tissue to the muscle.

Best thing you can do is continue taking your antibiotics as prescribed and have your wound repacked on a daily basis..

You never did say what antibiotic you were taking?

Could possibly have an excision :(

I went to the surgeon today, and I dont have to repack my wound anymore, its a good thing so it can heal but it feels nasty not having anything there to suck up the drainage except a man-sized diaper and tape.

An excision is basically them cutting out a diamond shape out of my ass after the infection has been cured so the re-occurrence of the cyst and infection are close to none.

The doc said he "guarantees" that the infection will come back, but ive heard its a 50-50 chance. I dont want to get this done, in 2 weeks I have to go back and get an evaluation and by then ill be back in school, and if I get the surgery ill be out of school for-fvcking-ever so sh1t guise! Idk what to do!!!:confused:

Missing a bit of school is a small price to pay for a healthy ass. Why take the chance the infection'll come back?

Well the healing process of an excision is around 4 months, thats a long time, I wont be missing school for 4 months straight. maybe a week tops but a week without turning in work is 6 straight fails in a row, no doubt at all.

Take some work home with you and study by yourself. You will more than likely learn more that way without people distracting you.

As far as infection who knows. 10g of vitamin C a day, coupled with 20g-30g of fish oil a day, and maybe rubbing TCP on the offending area regularly. Good luck.

But yeah, 10g+ of vit. C a day, 15-45g fish oil per day, and do what the doctor says, that's the best course of action. Pretty much what Quag said.

Ask your parents to get you a tutor.

It may also be helpful to get a second opinion. I went to three different doctors before I was diagnosed with cancer. I have always found most doctors to be stupid and unconcerned with your health.

This is gross. Please go have the surgery already instead of dealing with an unnecessary pain and grossness.

I got the OK from 2 doctors and 1 surgeon, Im pretty sure it is a Pilonidal Cyst or else they wouldnt have operated on it to begin with :)

LOL sorry your personal fantasies didnt happen to me :p

After the drainage there was no more pain, they gave me 2 painkillers and now theyre just collecting dust. the main reason for the removal of the whole thing is to prevent recurrence!

Ehh, hopefully not. Im not looking forward to it thats for sure. I just began the healing process, and if they excise the wound while the infection is still present, ill just develop something worse, says the surgeon. My main problem is the healing time if I get the excision, which takes 4 or more months.

Im waiting to heal up completely then ill see what im going to do from then, no more re-packing so my wound can heal from the inside out. Doc said he guarantees ill develop another cyst, why he "guarantees" it is beyond me...theres a 50-50 chance or something like that according to pilonidal.org, which is like a support site :crazy:

So yeah, im healing up pretty nicely right now, I still need to figure out when my skin around there is going to be as strong as it once was, so I can begin lifting again.
