I listened to the whole thing.
1. Ehh you thinking your voice sounding weird is just self-consciousness. I don't think your voice sounded weird. You sounded cool.
2. Too much bantering and not enough personal connecting, I think. This chick was at work, so you only had a couple minutes reasonably to talk to her before she had to get back to work. In the whole conversation, you joked back and forth but didn't make it personal at all, didn't get to know each other at all. Even in limited time, you should at least do some qualification.
3. I got the feeling that you reneged when asking for her number by quickly reacting and saying "Why are you laughing?" It kind of made it seem like you were unsure of yourself in asking for her #.
Anyway, really, really good job. First approach in 6 months? You sounded warmed up already. And remember, getting through those awkward situations is a very GOOD thing...kind of like pain is weakness leaving the body, well feeling awkward situations is equal to social inadequacy leaving your personality

Top 3 things you're most interested in wasn't very good. It's a bad interview question. Still, it kept the conversation a little sexual, and that's good.
